This notice describes how medical and mental health information about you may be used and disclosed, and how you can access this information. Please review it carefully.
1. Our Legal Duty
Embodied Soul Psychotherapy is required by law to protect the privacy of your protected health information (PHI), provide this notice of our privacy practices, and abide by the terms of this notice.
2. How We Use and Share Your Health Information
We may use or share your information without your written authorization for the following reasons:
Treatment – To provide, coordinate, or manage your care (e.g., consultation with another provider).
Payment – To bill and receive payment from insurance or other payers.
Health Care Operations – For administrative, quality assurance, or business management purposes.
3. Uses and Disclosures Requiring Your Authorization
We will ask for your written permission before:
Sharing psychotherapy notes
Using your information for marketing purposes
Selling your information
You may revoke your authorization at any time in writing.
4. Uses and Disclosures Without Your Authorization
We may also disclose your PHI without your consent in the following situations:
Child, Elder, or Dependent Adult Abuse – Mandated reporting to protective services
Serious Threat to Health or Safety – If necessary to prevent harm
Judicial or Administrative Proceedings – If ordered by a court
Health Oversight Activities – When required by a licensing board
Worker’s Compensation – As permitted by law
5. Your Rights as a Client
You have the right to:
Request limits on how we use or share your PHI
Request confidential communication (e.g., contact you at a specific phone number)
Access and get a copy of your health records
Request corrections to your health information
Get a list of times we've shared your information
Receive a paper or digital copy of this notice
Be notified of a breach involving your PHI
6. Additional Protections for California Residents
In addition to HIPAA, California’s CMIA, CCPA, and CPRA laws provide further rights:
The right to know what medical and personal information is collected
The right to request deletion of information
The right to opt out of data sharing or sale (note: we do not sell your data)
The right to non-discrimination when exercising your privacy rights

Confidentiality of Electronic Communications
While we take reasonable steps to secure email and form communication, please be aware that:
Unencrypted email is not fully secure
Online form submission does not create a therapeutic relationship
Secure video sessions are conducted through HIPAA-compliant platforms

Good Faith Estimate
Under the No Surprises Act, you have the right to a Good Faith Estimate of the cost of services if you are not using insurance.
If you receive a bill $400+ above your estimate, you may dispute it.
